My staff used to cut and paste from consents 20 years old to fill out the lay terms as they prepared consents. So I created a behemoth of a pdf form that wrote out the lay terms I wanted when they checked the procedures I recommended in my consult. No database! I wrote a java script for every single term coupled with a grammatically correct translation into lay terms. There were scripts for punctuation, different areas of the body, preventing blank spaces, and qr-codes. Oh, god, the qr-codes☹😖😤ðŸ˜ðŸ˜¨ðŸ˜©ðŸ¤¬!!! It was my way of providing the portability in HIPAA. Anyway, this damn pdf form got so unwieldy that I decided to make it into a database, and, because I hadn’t created an online database since mySQL in 2003, I decided to share with the world. Maybe even get other HCP’s to contribute. I’m a cosmetic surgeon, so that is where this starts. Hopefully, other specialties will jump aboard.
And just because json makes it possible, qr-codes are included.
